SD Card Format Error

Bought a micro SD card and worked fine for a couple of days.

A few days later it shows SD card capacity as 0m (Zero) , and unable to reformat SD Card.

I have tried to reformat the SD on my PC and still doesn't work.

Anyone had a similar issue? if so, how was it resolved?
